NPY Women’s Council (NPYWC) is an Anangu-led organisation governed by women’s law, authority and culture. We deliver health, social and cultural services for Anangu across the Ngaanyatjarra, Pitjantjatjara and Yankunytjatjara (NPY) region.

Our Tjungu Team provides support, advocacy and case management to people with disabilities, older people, their carers and families across 26 Aboriginal communities in South Australia, Western Australia and the Northern Territory.

We are seeking an experienced Clinical Team Leader – ATSI Aged Care Assessment Service to provide clinical leadership, guidance and supervision to our Aged Care Assessment staff.

About the Role

Based in Alice Springs with regular travel across the NPY region, the Clinical Team Leader will provide oversight of clinical and non-clinical aged care assessments while supporting the delivery of high-quality, culturally safe and trauma-informed services.

You will provide advanced comprehensive assessments and care planning, support staff through clinical guidance and supervision, and build productive relationships with external stakeholders to establish integrated care pathways for older people.

Key Responsibilities

  • Provide specialist comprehensive aged care assessments in accordance with relevant Commonwealth guidelines.

  • Develop appropriate care plans in partnership with clients and carers.

  • Provide clinical guidance, support and supervision to Aged Care Assessment staff.

  • Follow up with clients to ensure recommended care plans remain appropriate and effective.

  • Advocate and liaise on behalf of clients and carers across the care continuum.

  • Maintain effective relationships with health professionals, service providers and other key stakeholders.

  • Participate in regional triaging for complex geriatric clients.

  • Contribute to the planning, coordination and delivery of the Aged Care Unit.

  • Provide education and consultancy support relating to the care of older people.

  • Support orientation, induction and mentoring of new team members.

  • Maintain appropriate records and statistical data.

  • Participate in quality assurance, professional development and aged care assessment training.

About You

You are an experienced health or allied health professional with strong clinical assessment and care-planning skills and a genuine commitment to supporting older people.

You will bring:

  • Advanced comprehensive assessment and care-planning experience across the aged care continuum.

  • Knowledge of the Aged Care Act and its application to Aged Care Assessment services.

  • Significant knowledge and skills in the delivery of the Aged Care Assessment Program.

  • Strong liaison, networking, written and verbal communication skills.

  • The ability to work effectively within a multidisciplinary team.

  • Strong computer skills, including Microsoft Office.

  • A commitment to culturally safe and trauma-informed practice.

Qualifications

You will hold tertiary qualifications in one of the following areas:

  • Occupational Therapy, Psychology or Nursing, with eligibility for registration with the relevant Board; or

  • Social Work, with eligibility for full membership of the Australian Association of Social Workers or the Society of Professional Social Workers.
