Ngaanyatjarra Pitjantjatjara Yankunytjatjara Women’s Council (NPYWC) is an Anangu-led organisation delivering health, social and cultural services across Central Australia.
We are seeking a dedicated Project Officer – Aged Care Assessments to join our Tjungu Team, supporting culturally safe aged care services for older Anangu people across remote communities.
About the Role
Based in Wingellina Community, Western Australia, this position supports the establishment and delivery of NPYWC's Aboriginal and Torres Strait Islander Aged Care Assessment Service.
You will undertake aged care assessments, develop individualised care plans, coordinate referrals and work closely with clients, carers, health professionals and community service providers.
The role involves regular travel across remote communities and is guided by culturally safe and trauma-informed practices.
Key Responsibilities

Undertake aged care assessments within required timeframes.

Develop appropriate care plans in consultation with clients and carers.

Coordinate referrals to relevant health and community services.

Follow up with clients to ensure care plans remain appropriate and effective.

Advocate for older Anangu people and their carers.

Build and maintain effective relationships with health professionals and service providers.

Maintain accurate, confidential client records and reporting requirements.

Participate in quality assurance, training and service improvement activities.

Contribute to Tjungu Team planning, coordination and service delivery.
About You
We are looking for someone who has:

Tertiary qualifications and/or demonstrated relevant experience in health, community services or a related field.

An understanding of the needs of older people, people with disabilities and their carers.

Strong communication and interpersonal skills, including the ability to work effectively with Anangu communities.

The ability to work independently, manage competing priorities and contribute to a team.

Good organisational, record-keeping and computer skills.

A current Australian driver's licence, with a manual licence required under NPYWC employment conditions.

A willingness to travel and work in remote communities.
